A Study of Partial Nephrectomy


Abstract
Althogh many kinds of operative techniques on the renal diseases have been developed, partial nephrectomy still remains one of the most difficult procedure. Maximal conser--vation of functioning renal tissue is the duty of urologist and major objection of surgical -treatment for the localized renal diseases.
A retrospective review was don3 on 44 consecutive partial nephrectomies on 42 patients :seen between 1964 and 1979.
The following results were obtained:
1) The range of patients age were 10 to 61 years and mean age was 40.2 years. There -were 35 male and 7 female patients.
2) Of the 44 partial nephrectomies, 24 cases were on the right side and 20 cases were -on the left side. The lower pole were removed in 30 cases and the upper pole were .removed in 14 cases.
3) The indications of partial nephrectomies were renal stone or stones in 39 cases, localized renal tuberculosis in 2 cases, renal injury with horse-shoe kidney in 1 case, ,double collecting system with ectopic ureteral orifice in vagina in 1 case and localized renal abscess in 1 case. One patient of multiple renal stones received bilateral partial nephrectomies and another patient of multiple renal stones received upper and lower partial nephrectomies on the right side at the same time.
4) Histopathology of resected specimen in 39 renal stone or stones natienia revealed chronic or healed pyelonephritis in 76.9o and microliths were fount in 4.1%.
5) Postoperative complications were seen in 10 cases.
i) 2 of 3 wound infection cases were the patients who had previous urinary tract infection, none of which needed surgical intervention.
ii) The ileus were noted in 2 cases and promptly relieved by insertion of Levine tube.
iii) Persistent urine leakage were seen in 2 cases. Spontaneous cessation of the urine
leakage took place on the 11th postoperative day in one case and ureterolithotomy
was needed in another case becase fragmented stone particle occluded ureter.
